To make an appointment for an exam in Radiology at UWMC or Roosevelt.  You will need to:

• Get a referral form from your doctor.  This needs to be faxed to 206-598-7690 prior to calling the scheduler.
• Then call 206-598-7200 to talk with a scheduler. At that time, you will discuss the available times for the exam.  The preparations for the exam will be explained to you.
• You will receive a reminder call about your appointment from the Radiology Department 48 hours before your exam.
• The scheduling office hours are Monday-Friday 8:00 am-5:00 pm.

If your doctor’s office wants to call to make the appointment, they will need to:

• Fax the referral form for the exam to 206-598-7690 prior to calling the scheduler.
• Then call the scheduler at 206-598-7200 to schedule an appointment and get the preparations for the exam.
• You will receive a reminder call about your appointment from the Radiology Department 48 hours before your exam.
