Finalize Proposal

Budget & Related Documents

For Industry-Sponsored Clinical Trials, include the approved CRBB budget.

For other proposal types, include a yearly detail budget that specifies:

  • Yearly salary, benefits, and FTE for all personnel.
  • Yearly rate, usage, and estimated total cost for each Radiology resource, such as MRI, CT, or PET (along with associated PET physics and radiochemistry charges). 

Include documentation from pricing focals and a copy of the approved AAA form. (See Obtain Pricing Documents).

Regulatory Approvals (Non-NIH Apps ONLY)

For non-NIH proposals, projects using human or animal subjects will require IRB or IACUC protocol numbers and approval dates (this is required for completion of the eGC1).  If PET or CT will be used, an approval letter from EHS will also be needed before OSP can approve the application.

 

If SFI exists, signed GIM10 forms should be present, along with supporting documentation in a sealed envelope.

 

Collaboration Materials

Ensure all final collaboration materials are accounted for, including current biosketches, letters of support, and complete, approved subcontract(s).

 

SOM Forms

The Office of the Dean, School of Medicine, requires that both the Scientific Code Classification Checklist School and the Medicine Grant Activity Sites Inventory forms be included in its copy of the proposal.  Both forms can be downloaded from the Researcher's Guide Forms Page.
