FIRST, read these documents and make sure that your proposed dates of leave are feasible, given the dates and the site of practice:

THEN, if everything should work out you can go on to fill out the Resident Leave Request Form.  Use the GME form if five or more days off are required for Sick/Maternity/Paternity.  You will need to be logged in to access this form.
